ADHD assessment: steps to assessment
These are the steps people can expect to go through with an ADHD clinic screening. The clinic team will explain what happens and what is needed at each stage. Face-to-face appointments are available in the North East of England, including Sunderland and Newcastle, and in Liverpool, with online or telephone appointments across the UK.
STEP 1
Referral Form
The person must complete and submit the online referral form.

If you need the referral form, contact the clinic team and they will direct you to it.
STEP 2
Triage
- All referrals go through triage before acceptance.
- The clinical team may decline referrals where more specialist support is required; a clinician will discuss the reasons directly.
- The QbTest is a Type 1A medical device subject to strict usage guidelines.
- Within 7 days of submitting the referral form, administration will contact the person if further information is needed.
- The referral form must be accurate and complete. Incomplete or withheld information can affect the outcome, prevent progression to Clinical or Associate teams, and may result in an inconclusive assessment.
STEP 3
Payment
After acceptance, an invoice is issued.
A £200 non-refundable deposit is required and comes off the full diagnosis cost.
Appointments cannot be booked until payment is made. Appointments are non-refundable, including if the person changes their mind.

Service 3 — Full Diagnosis is £1,090 and includes the screening and Qb test.
Where the page mentions paying in stages, the three services and their prices above apply, with the formal diagnosis total £1,090. The staged £650 option is the screening plus QbCheck and is not a formal diagnosis. It is a supplied package price.

STEP 4
Full Assessment
- After payment, a link is sent to book an appointment slot.
- The full screening appointment lasts approximately 2 hours.
- In person: QbTest plus ACE and DIVA-5 interview.
- Remote: QbCheck plus ACE and DIVA-5 interview.
- After the QbTest/QbCheck there is a short break before the interviews.
- ADHD rating scales are sent in advance.
- The results report is not completed until all required questionnaires are returned and payments are received.
QbTest Only
- No rating scales or diagnostic interviews are sent/included.
- A 2-hour slot is allocated, but the appointment itself is completed within 1 hour.
- The person cannot be referred to clinical associates if they have only completed QbTest.
STEP 5
Screening Results
- The clinical team compiles a report from all assessment data.
- The report is sent within 4 weeks after the appointment.
- This is a screening outcome, not a formal diagnosis.
- If results suggest ADHD, the person can decide whether to pursue formal diagnosis.
- Referral to the next stage is possible only after the full assessment is completed: QbTest/QbCheck, ACE interview and rating scales.
If someone completed QbTest only and later chooses formal diagnosis, the supplied additional charge is £700.00 for the remaining elements of Service 1+3, including clinical interview and rating scales, and a new more comprehensive report.
The clinic should confirm how the £700 relates to the £1,090 total before publication if necessary. Please confirm current fees with the clinic before booking.
